THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L. c.111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. The list of participating providers is at www.careingtonrxcard.com and www.amplifonusa.com/careington. A written list of participating providers is available upon request. You may cancel within the first 30 days after effective date or receipt of membership materials (whichever is later) and receive a full refund. Ambulance and Paramedic Expense, Auto Insurance Deductible Reimbursement, Child Accident Expense, Emergency Medical Expense Reimbursement, Home Insurance Deductible Expense Reimbursement benefits are provided under a blanket policy issued by American Bankers Insurance Company of Florida to Home Benefits, LLC. Loss must occur while plan is in effect. They are subject to terms, conditions, and exclusions.

Auto Insurance Deductible Reimbursement in Texas, Auto Repair Reimbursement, and in Canada -Ambulance & Paramedic Reimbursement, Emergency Medical Expense Reimbursement, Home Insurance Deductible Reimbursement benefits are provided under a policy from an A rated insurer. Loss must occur while plan is in effect. They are subject to terms, conditions, and exclusions.

Due to state restrictions:
• $2,000 Legal Defense is not available in CA
• Bail Bond is not available in CA & NY
• $2,000 Uninsured Motorist Protection is limited in NY and not available in CA & NV

The following benefits are not deemed as Emergency in MD:
• Emergency Medical Expense Benefit
• Emergency Transportation and Car Rental Expense Benefit
• Emergency Living Expense Benefit
• Emergency Cash Wiring Fee Reimbursement
